Under agreements reached with the Department of Justice, at least six major hospitals have undertaken to withhold gender-affirming hormones and surgeries from transgender young people, and have consented to make payments to the federal government.

The agreement reached in recent days was only finalized last week.

The vast majority of hospitals had already ceased providing such treatments, whether in response to state-level bans or to prior pressure exerted by President Donald Trump's administration.

The agreement reached by the Department of Justice both terminates the investigation into care previously provided and eliminates the possibility that hospitals could be compelled to hand over patient records.

The Department of Justice stated that some healthcare providers have also agreed to help cover the medical costs of children's "detransition."

None of these hospitals admitted to any wrongdoing.

"If the federal government is permitted to politicize and then terminate health care for any group, it would set an extraordinarily dangerous precedent," said Eliel Cruz, a co-founder of the Gender Liberation Movement.

The following is a summary of the situation:

The Department of Justice announced last week that it had reached settlements with NYU Langone Health and the University of Pittsburgh Medical Center.

The aforementioned medical institutions agreed to cease providing puberty blockers, hormones, or gender-affirming surgery to patients under the age of 19 — interventions that the government collectively terms "gender-denying surgeries."

"The Department of Justice is committed to defending our nation's children and is working tirelessly to secure agreements with hospitals to put an end to so-called 'gender-affirming care' for minors," U.S. Attorney General Todd Blanche said last week in a statement.

New York University has agreed to pay the federal government $8.5 million, while UPMC has agreed to pay $950,000; these agreements were concluded in the wake of a series of similar settlements reached since May with the Cleveland Clinic, Connecticut Children's Hospital, the Mount Sinai Health System in New York, and Texas Children's Hospital.

These hospitals asserted that the aforementioned agreements would have no bearing whatsoever on the manner in which their pediatric mental health programs approach the treatment of transgender children.

A review report issued last year by the Trump administration urged that adolescents be treated solely with psychotherapy, rather than with the more comprehensive approach of gender-affirming care.

However, most major medical organizations, including the American Medical Association and the American Academy of Pediatrics, support the continued provision of such treatment.

Mount Sinai is the only one among the six institutions that have so far reached agreements that had not previously disclosed its decision.

Hospitals in Texas and Ohio have suspended the provision of such care in compliance with state-level bans.

Since Trump returned to the White House in January 2025 and initiated a series of measures targeting such care, other hospitals have likewise announced policy adjustments in turn.

In January 2025, he signed an executive order calling for a reduction in federal support for gender transition among individuals under the age of 19, and the government has since prohibited federal Medicaid funds from covering such treatment.

Such care remains accessible through a diverse array of hospitals, clinics, and individual physicians.

One pivotal measure undertaken by the government was to subpoena hospitals—a move first announced in July 2025—compelling them to surrender documentation pertaining to gender-affirming care for children, inclusive of patients' names.

NYU Langone Medical Center declared in a statement that the settlement reached on this occasion affords comprehensive protection to patients and their families, in that it has dispelled the apprehension that "NYU Langone would be compelled to disclose confidential information, previously demanded, concerning patients under the age of 18 who have received gender-affirming care at this institution."

Several hospitals have petitioned the courts to block the compelled turnover of records, and most of them prevailed at the district court level.

However, a ruling handed down by the appellate court last month has undoubtedly injected formidable momentum into the government's litigation position.

The United States Court of Appeals for the Ninth Circuit, sitting in San Francisco, ruled by a two-to-one vote that the government's termination of gender-affirming care for minors was not improper in its intent and therefore did not suffice to justify quashing the subpoena.

At least 27 states have successively enacted laws that restrict or even expressly prohibit minors from receiving gender-affirming care.

Moreover, since last year, in states that have not enacted bans, more than two dozen hospitals, health systems, and clinics have ceased providing such care—including no small number of states that have enshrined specific legal protections for it.

A group of Democratic state officials has filed suit against the Trump administration over this matter, alleging that its actions amount to unlawful intimidation of health care providers.

Gender reassignment surgery before puberty is exceedingly rare; nevertheless, a great many transgender adolescents undergo hormonal interventions in order to halt the progression of puberty or to bring about the development of sexual characteristics consonant with their gender identity.

Cruz's gender liberation movement has already staged protests against the hospitals that have discontinued this care.

Cruz says that some families have relocated to other states in search of treatment, only to discover that providers there, too, have ceased offering it.

He says that some have left the country altogether.

He asserted that the agreement reached with the Trump administration made plain "who possesses the courage to carry out what their mission and their education summon them to do—namely, to do no harm—and who does not possess such courage."

This article has been updated to correct the following: the agreement required hospitals to make payments to the federal government, not for the government to pay in order to reverse gender transitions.
